    To submit a student transfer request, please locate the appropriate form for the student from the options below.

    NOTE:  YOU WILL RECEIVE NOTIFICATION OF REQUEST STATUS VIA EMAIL. (NOT SEEING IT?  CHECK YOUR SPAM FOLDER.) UPON RECEIVING APPROVAL, YOU MUST DOWNLOAD AND PRINT A COPY OF THE PDF TO TURN IN TO THE DISTRICT YOU WANT YOUR CHILD TO ATTEND.

    Secondary Verification of Student History (7-12)

    NOTE: A release form from your resident district must be submitted with all (K-12) Nonresident Transfer Request forms. When submitting a Secondary Nonresident Transfer Request, you must also attach a Verification of Student History (grades 7-12 only) in addition to the release from your resident district.  If your student (grades 7-12) is currently attending a Bethel school, the Verification of Student History is not necessary.
