ALL parents of student-athletes must use FinalForms to complete and sign athletic participation forms for your students.
FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
IMPORTANT: You will receive an account confirmation email from FinalForms. Please click the link and follow the steps to login and access your student(s). If you require any support during the process, scroll to the page bottom and click “Get Help”.
If you cannot locate the email from FinalForms, please contact the Athletic Director at your school.
Art Crate Guidelines
To ensure the safety of our students, parents, and community, the following must be adhered to:
Only clear bags or small clutch/purses/wallets will be allowed inside the stadium. Bags must be clear plastic, vinyl, or PVC and do not exceed 12” x 6” x 12”. A small clutch bag, not exceeding 4.5” x 6.5”, with or without a handle or strap, may be carried inside the stadium.
Additionally, all elementary and middle school students who plan to attend school-sponsored events, such as football games, must be accompanied by an adult. They will be turned away if they attempt to gain admittance without a parent.

Hall of Fame
BSD Athletic Hall of Fame
The Bethel School District Athletic Hall of Fame, an independent non-profit organization, was started to ensure that the athletic history and traditions of the district are celebrated.
Their mission is to honor past teams, athletes, coaches and contributors of Bethel School District.
Board Meetings are held on the first Monday of every month at noon at the Bethel Learning Center.
